消瘰冲剂联合抗结核药物治疗颈部淋巴结结核患者的疗效观察

Efficacy of Xiaoluo Granules combined with antituberculosis drugs in treatment of patients with cervical lymph node tuberculosis

    目的 探讨消瘰冲剂联合抗结核药物治疗颈部淋巴结结核患者的临床疗效。
    方法 回顾性分析2021年1月—2023年1月在本院接受治疗的118例颈部淋巴结结核患者的临床资料。根据治疗方式不同,将患者分为对照组(57例,接受常规抗结核药物治疗)和观察组(61例,接受消瘰冲剂联合抗结核药物治疗)。比较2组疗效、中医症状积分、外周血T细胞亚群水平以及辅助性T细胞1型/2型(Th1/Th2)相关细胞因子水平,包括肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)和干扰素-γ(IFN-γ)。
    结果 治疗6个月后,观察组治疗总有效率高于对照组,差异有统计学意义(P < 0.05)。治疗6个月后, 2组疼痛、低热、乏力、厌食及盗汗症状积分均下降,且观察组上述症状积分均低于对照组,差异有统计学意义(P < 0.05)。治疗6个月后, 2组CD3+、CD4+及CD4+/CD8+均升高, CD8+均下降, 且观察组CD3+、CD4+及CD4+/CD8+高于对照组, CD8+低于对照组, 差异有统计学意义(P < 0.05)。治疗6个月后, 2组TNF-α、IL-6、IFN-γ水平均下降, IL-10水平上升,且观察组TNF-α、IL-6、IFN-γ水平低于对照组, IL-10水平高于对照组,差异有统计学意义(P < 0.05)。
    结论 消瘰冲剂联合抗结核药物治疗颈部淋巴结结核能显著提高疗效,并通过上调CD3+、CD4+、CD4+/CD8+和IL-10水平, 下调CD8+、TNF-α、IL-6、IFN-γ水平来增强机体免疫功能。

     

    Abstract:
    Objective To investigate the clinical efficacy of Xiaoluo Granules combined with antituberculosis drugs in the treatment of patients with cervical lymph node tuberculosis.
    Methods A retrospective analysis was conducted on the clinical data of 118 patients with cervical lymph node tuberculosis who were treated in our hospital from January 2021 to January 2023. The patients were divided into control group (57 patients receiving conventional antituberculosis drug treatment) and observation group (61 patients receiving Xiaoluo Granules combined with antituberculosis drug treatment) based on their treatment methods. The efficacy, traditional Chinese medicine (TCM) symptom scores, peripheral blood T-cell subsets, and type 1/type 2 helper T cell (Th1/Th2)-related cytokine levels, including tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-10 (IL-10), and interferon-γ (IFN-γ), were compared between the two groups.
    Results After 6 months of treatment, the total effective rate in the observation group was higher than that in the control group(P < 0.05). After 6 months of treatment, the scores for pain, low fever, fatigue, anorexia, and night sweats decreased in both groups, and the scores for these symptoms in the observation group were lower than those in the control group (P < 0.05). After 6 months of treatment, CD3+, CD4+, and CD4+/CD8+ increased, while CD8+ decreased in both groups. Additionally, CD3+, CD4+, and CD4+/CD8+ were higher, while CD8+ was lower in the observation group than those in the control group (P < 0.05). After 6 months of treatment, TNF-α, IL-6, and IFN-γ levels decreased, while IL-10 levels increased in both groups. Furthermore, TNF-α, IL-6, and IFN-γ levels were lower in the observation group than in the control group, while IL-10 levels were higher(P < 0.05).
    Conclusion Xiaoluo Granules combined with antituberculosis drugs can significantly improve the efficacy in the treatment of cervical lymph node tuberculosis and enhance immune function by upregulating CD3+, CD4+, CD4+/CD8+, and IL-10 levels and downregulating CD8+, TNF-α, IL-6, and IFN-γ levels.
